We are seeking a professional, compassionate, and highly organized Client Liaison Officer to join our team on a maternity leave cover basis, with the possibility of becoming a permanent role for the right candidate. In this pivotal position, you will ensure a seamless client experience by providing high-quality support and acting as the key point of contact for client inquiries, care coordination, and service delivery. The Client Liaison Officer will be responsible for:Provide empathetic, solution-driven support to elderly and disabled clients, fostering trust and confidence in our services. Handle daily client inquiries, feedback, and complaints with professionalism and care. Serve as the main point of contact between clients, families, referral sources, carers, and administrative staff, ensuring seamless service delivery. Educate clients and families on available services and care plan benefits, ensuring clarity and understanding. Approve the supply of medical devices and aids, coordinating with suppliers to meet client needs. Oversee recruitment, orientation, and retention of care teams, ensuring efficient operations. Collaborate with the accounts department to ensure accurate billing aligned with care plans. Manage and track client inquiries and sales opportunities, keeping the CRM system up-to-date. Stay informed on industry trends in aged care, disability services, and health sector developments. Minimum Requirements:Diploma or Bachelor's degree in Aged Care, Community Services, or a related field. Proven experience in care coordination or case management, particularly within the aged care sector. Strong knowledge of aged care services, including Home Care Packages and NDIS. Ability to develop and manage person-centered care plans that meet individual client needs. Excellent interpersonal and communication skills, both written and verbal, with a focus on active listening and problem-solving. Proficiency in using CRM systems and maintaining accurate client records. Familiarity with health, disability, and aged care regulations and best practices. Valid First Aid and CPR certifications. Current National Police Check. Full driver's license and the right to work in Australia. Nursing qualifications or a clinical background would be highly regarded. Experience in conducting clinical assessments and tailoring care plans to meet the specific needs of clients. Experience in the recruitment, training, and management of care teams.
