Reason for Coverage: New program growth Dates of Coverage: Oct 7 2024 for 10 months Call Ratio: 1:5 Patient Population for Required Coverage: Adult Geriatric Percentage of Practice: Assume 50% Hematology and 50% Oncology Number of Physicians in Practice: 7 across 2 locations APPs Provide Coverage: 10 Average Number of Outpatient Per Day: 1015 Average Number of Hospital Patients Per Day: 15 Clinical Area: Oncology Outpatient Trauma Level/Designation: Level 2 Trauma Documentation System/EMR: Epic Procedures Required Chemotherapy Intrathecal Chemotherapy Intracavitary Intrplueral/Peritoneal FNA/Biopsy Mgmt.

of VAD/CV Catheters Administration of Chemotherapy: Physician orders.
