What You'll Do:Administer effective physical therapy treatments according to physician's orders, ensuring optimal outcomes for residents.

Evaluate residents' conditions, create customized care plans, and maintain detailed progress notes, evaluation summaries, and discharge summaries.

Collaborate with other healthcare team members to develop strategies that promote independence or maintain current levels of functioning.

Participate in Resident Care Planning meetings, offering valuable input on resident improvement plans or strategies.

Step in to represent the Physical Therapy Department at care meetings during the Director of Physical Therapy's absence.

Maintain accurate statistical records of residents' treatments for internal and regulatory purposes.

Who Will Be a Great Fit:Master's or Doctorate degree in Physical Therapy (MS/DPT).

Active and valid New York State Physical Therapy license in good standing.

At least one year of experience as a Physical Therapist in a Skilled Nursing Facility is preferred.
