Continuum Care Hospice, an affiliate of Legacy Health Services, is looking to hire an experienced and dedicated RN Hospice Case Manager to work with our team of professionals.

The ideal candidate will be a Registered Nurse with at least one (1) year of experience.

Hospice experience is a plus.

Hospice is not a place; it's there when needed, where needed, whether it's a private residence, nursing facility, or assisted living.

Our professional hospice team works with our residents, their family, and physicians to provide medical, psycho-social, emotional, and spiritual care.
