Job Title: Special Needs Coordinator
Location: Aviano Air Base, Pordenone, Italy
Duties: Use clinical assessment skills and maintain procedures to identify sponsors whose family members have special medical and educational needs in a timely manner, such as installation youth and childcare facilities, and officer and enlisted spouses' groups. Independently assess the psychosocial functioning and needs of patients and their family members and formulate and implement a treatment plan. Ensure all active-duty sponsors known to the Air Force Personnel Center as having family members with special needs are identified in Air Force medical special needs data management systems. Provide clinical training to installation personnel and medical staff. Collaborate with the Integrated Delivery System and other interagency forums that provide family medical information and referrals to base and civilian agencies. Provide information, appropriate contact information, and coordinate referrals as appropriate. Participate in EFMP quarterly case reviews to discuss and assess newly identified families' complex, or unmet clinical needs. Determine the appropriate resources necessary for the families. Participate in the clinic orientation and training of other staff of the Special Needs Program. May serve on committees, work groups, and task forces at the facility. Complete medical record documentation and coding, and designated tracking logs and data reporting, policies, and guidance. Education and Qualifications: Graduate of associates (ADN) or baccalaureate degree (BSN) program in nursing accredited by a national nursing accrediting agency recognized by the US Department of Education. A BSN is preferred. Maintain an active, valid, current, and unrestricted license to practice nursing as a registered nurse in any US state/jurisdiction. Preferred a minimum of two years full-time experience in care coordination, case management or discharge planning experience in a medical setting within the last four years. Current Basic Life Support certification is a job requirement. Language Requirement: Read, understand, speak, and write English fluently. Must have or be able to pass a T. 1 background check. Must also have or be able to pass a National Agency Check. Knowledgeable in medical privacy and confidentiality (Health Insurance Portability and Accountability Act); accreditation standards of Accreditation Association for Ambulatory Health Care and The Joint Commission. Ability to input, extract and format data from the Armed Forces Health Longitudinal Technology and Composite Healthcare Computer System. Skillful and tactful in communicating with people who may be physically or mentally ill, uncooperative, fearful, emotionally distraught, and occasionally dangerous. Organization, problem-solving and communication skills to articulate medical requirements to patients, families/caregivers, medical and non-medical staff in a professional way.
